Once you have scheduled an appointment, you will receive a client portal invitation to complete Valley Psychological Service's documents. If you prefer you may complete the documents in the office. Please allow 1/2 hour prior to your first appointment if you choose to complete the documents on site. Please bring your list of medications of your first appointment.
